A DMTx experience is well within an acceptable safety profile when done correctly. Here are the categories we evaluate.
The original study conducted by Dr. Rick Strassman, author of DMT: The Spirit Molecule, in the early 1990s was a Phase 1 safety study. Dr. Strassman showed that DMT could be administered using an intravenous shot, even multiple times in a row, without any significant physiological issues. Since this time, multiple studies, including DMTx studies, have shown that DMT can be administered safely in this setting.
We have conducted 33 DMTx sessions in Boulder and have had no physical safety issues. With that being said, our medical team is skilled in administering the IV, the infusion, and skilled in medical emergencies. We have an AED, epipen, and other medical safety equipment on site. The hospital in Boulder is less than 5 minutes away.
The N,N-DMT of the DMTx Project has been extracted from Mimosa hostilis root bark, in compliance with the Natural Medicine Health Act of Colorado. Our sacrament is not synthetic and contains trace amounts of NMT, another tryptamine and precursor to N,N-DMT. It was created in a sterile lab by skilled chemists, and tested by this lab and two independent labs for both potency and purity. No endotoxins are present and it has been used previously without any problems. Administering the sacrament through an IV poses the same risks as any IV infusion and are low; however, the IV start may require more than one attempt.
While the physical safety profile is high, we want prospective clients to be aware that the real risks associated with DMT are related to mental health, particularly the overuse of DMT. It is imperative that you are honest in the safety screening process and disclose all mental health concerns. This practice may not be safe for people with symptoms of psychosis or bipolar disorder, among others. These risks are mitigated by good screening practices, and only allowing those who are ready for it to have such a profound experience. Significant overuse of DMT is known to cause mental health concerns, and this is sometimes evident in recreational users in the larger psychedelic community.
Overwhelm in the journey itself is also something to manage. This is why having a skilled, advanced psychedelic guide practitioner with you the entire time is so important. Daniel McQueen, as a psychedelic specialist, has been a professional, aboveboard (not underground) guide for the last 14 years, in private practice in Boulder, Colorado. He solely works in legal settings. He's been trained to lead these sessions for the last 9 years, and knows how to provide the utmost support during these experiences by matching the level of the experience with what the journeyer can tolerate. This is not a one-size-fits-all experience, nor does it have to be. A skilled guide — not just medical support — represents the highest standard of psychedelic care for the DMTx experience.
Sometimes, it's not the journey itself, but the implications of the journey, that are the most difficult to navigate. This is why ongoing support through the one-month integration period is so essential. We also invite journeyers to join our church community for ongoing integration support over the following year. Here are a few things to consider:
Psychedelic Overvisioning — Trying to produce a big idea right away without proper integration can lead to real difficulties in our lives. While the vision may be real, translating it into the material world often takes more time than we think it should, leading to overcommitting or trying to implement plans before we are ready. This may lead to decisions with real life or financial consequences. This is a primary reason that working with a skilled guide during the integration period is so important.
First Contact Existential Anxiety — Meeting entities, angels, or aliens for the first time in a space that feels "more real than real," and unpacking the messages they bring us, can elicit a state of anxiety as we establish a new world view to fit new understandings. This just takes time, and sometimes additional guidance.
Overuse of Psychedelic Medicines Before & After the DMTx Experience — We request that our journeyers participate in what our community calls a "medicine fast" for one month before and after your DMTx retreat. We want you to be as grounded as possible before the expedition, and to give yourself an opportunity to fully "land" after the DMTx journey. It has been our experience that this integration period takes about a month. Again, psychedelic medicines are very safe, but they are powerful tools best used with intention. This is not a recreational experience. Be prepared for the unexpected and give yourself the greatest gift you can. This means slowing down, caring for self, and not overusing psychedelic medicines.
There are always inherent risks associated with taking any psychedelic medicine, including DMTx. However, the safety of both the medicine itself and the process is very well established. No other medicine experience can be "turned off" in a matter of a few minutes if someone is having a difficult or overwhelming experience.
With that being said, a DMTx experience is best described legally speaking as a form of "Adventure Tourism" with inherent risks. To participate, you will sign Informed Consent documents, participation agreements (guidelines we all agree on to keep you safe), and a release & waiver of liability form — just as you would if you were exploring any other psychedelic, or going on a safari tour. This is standard practice in aboveboard, legitimate settings. While there will always be inherent risks, DMTx is way safer than a trip to the International Space Station or the Titanic.

You should be interviewing your guide as much as they're interviewing you. Can others vouch for them? Do you trust them, or are you just excited? Your "yes" should be a "hell yes."
Self-taught is no longer acceptable. Being told by "Spirit" that one is a guide is the first step on a long road. DMTx requires real training. Look for community accountability and verifiable credentials.
Was it extracted in a real lab or someone's kitchen? Was it tested for impurities and endotoxins? Because the sacrament is administered through an IV, lab verification matters enormously.
If your guide doesn't do thorough medical and mental health screening, that's a major red flag. Honest disclosure protects you.
Those with a mindfulness or self-exploration practice beyond psychedelics get the most out of DMTx. Physical exercise increases endurance.
Are you ready? Not excited — ready? Only you can answer this. DMTx is, by its nature, potentially life-transforming.
First and foremost, the Center for Medicinal Mindfulness is a Psychedelic Education and Harm Reduction Program. We believe informed consent is a primary ethical practice and we want our clients to be as informed as possible about our services, the risks involved, and the risks that can occur in working with others.
The concept of DMTx has inspired a lot of people, including guides who work in the underground. Please consider these factors in determining who you work with, and how to mitigate or manage the inherent risks associated with this experience. Infusing DMT into the bloodstream is a significant decision and must be done with great care. While we respect the inspiration, we want people to stay safe, and to do this work correctly.
Know your guide. You should be interviewing your guide as much as they are interviewing you. Develop a relationship of real trust, not just excitement, for the process. Do you trust and know your guide, or are you just meeting them before your journey? Can others vouch for your guide? How do you know they are safe? How do you know the container of the experience is safe? Your guide should be able to answer all of your questions with patience and respect and never try to persuade you to do this work, or dismiss your concerns. Don't dismiss your concerns. Your "yes" should be a "hell yes," with all concerns addressed.
Understand your guide's experience level and training. Does your guide have the training, experience, and skill to best support you? There are training programs, apprentice programs, and specialty trainings now. Solely being "self-trained" is no longer an acceptable level of support. Being told by "Spirit" or the "medicine" that one is a guide is just the first step on a long road to being an ethical guide. DMTx requires real training. Does your guide have this training, and can it be verified? Does your guide have community accountability and support? Or are they on their own? Big visions and messianic language may be significant warning signs that your guide is unsafe and may be overusing psychedelic medicines.
Know the substance. This is one of the most important aspects given the infusion nature of the experience. Was the medicine extracted or synthesized in a real lab, or someone's kitchen? Was it tested for potency, impurities, endotoxins, and chemical contaminants? Or was it just filtered with no lab testing? A lot of underground practitioners are making or extracting this in their kitchens, and some of them don't know what they don't know when it comes to substance safety. Because the sacrament is being administered through an injection, verifying that it's been tested by a lab is extremely important. Our sacrament has been tested by the extraction lab and two other independent labs to ensure its purity, potency, and safety.
Be honest in the screening. Most concerns can be safely managed if we know about them. It's important to be honest in the screening and intake forms so we can get to know you, your history, and your process. We can only help if we know what's going on and we want to make sure this experience is right for you. If your guide does not have adequate screening, or know how to do both medical and mental health screening, this is a red flag.
Train for the experience. We will prepare you as best we can through the preparation sessions, and have provided the DMTx Psychonaut Training series for you to review. It has been our experience that those with a mindfulness or self-exploration practice beyond taking psychedelics get the most out of these experiences, and can navigate the biggest waves this experience elicits. Physical exercise also helps increase endurance.
Be honest with yourself. Are you really ready? Not just excited, but ready? Only you can answer this question. Be honest with yourself and your guide. DMTx is, by its nature, potentially life-transforming.
We ask our psychonauts to fast from psychedelic medicines (cannabis is OK; alcohol moderate or none) for one month before and one month after your retreat. We want you grounded going in, and grounded coming out. The integration period takes about a month.
